Up-to-3 Early Intervention

Directions to Resource: Our Logan office is located in the Center for Persons with Disabilities on USU campus. We also have an office in Brigham City on 400 East and Forest Street. Most of our services and evaluations are done in your home, so you can call if you need directions to our sites.

Hours of Operation: 8:00am-5:00pm, Monday-Friday.

Criteria for Receiving Services: Please call us to help us decide which team members should come to your home to evaluate your child. If your child has enough of a delay or a medical diagnosis that affects development, they can be eligible to receive early intervention services.

Fees: There is no fee for evaluation. Medicaid covers our services or there is a sliding scale fee based on the family's income and number in the family. The fee is per month, no matter how many services you receive that month.

Additional Information: Up-to-3 provides intervention and parent training for families who have a child under 3-years-old with a developmental delay or a disability. We provide the evaluations and services in the child's home, unless another site fits the family's needs better. We also help link families to other service providers they may need.
