 Hours of Operation: Monday-Thursday, 8:00am-6:00pm; Friday, 8:00am- 4:00pm

Procedure for Connecting with the Service: Call (435)752-0750 or come into our outpatient center during regular business hours in order to schedule an initial assessment appointment.

Fees: Free services for traditional Medicaid card-holders. A sliding-fee scale is available for individuals who qualify as Severely Persistently Mentally Ill (SPMI) or Severely and Emotionally Disabled (SED).

 Additional Information: We serve English and Spanish languages and additional languages by arrangement.
